Sub–Muscular vs. Sub–Glandular

A common question our breast augmentation patients ask is should I go under the muscle or above the muscle? It is best for patients to come in for their free consultation to determine which procedure is best for them. There are both pros and cons for each procedure.

Sub-glandular breast implant placement: the implants are placed directly on top of the pectoral muscle and just behind the breast tissue.  Some women prefer this placement because they are easier to place by the surgeon, support larger implants, require less recovery time and post-op discomfort and can create more cleavage. On the other hand, some women prefer not to get sub-glandular breast implants because they tend to have a more augmented look, are more susceptible to rippling (if using a saline implant), and can sometimes complicate mammogram readings. Usually, it comes down to the woman’s preference of look and feels for this type of placement.

Sub-muscular breast implant placement: the implants are placed beneath the pectoral muscle, and sometimes only partially beneath the pectoral muscle using specific incisions and placement on the surgeon’s part. This type of placement is ideal for those who want to avoid the risks of having the implants ripple as well as have more successful mammogram readings.  On the other hand, sub-muscular implants are a more technical procedure, requiring a longer healing time, more post-op discomfort, as well as the risk of distorting when the muscle is flexed, and less easily creating cleavage, especially if the implants are placed farther apart.

Fat Transfer – From your thighs to your breast!

Most plastic surgeons have performed liposuction for years and fat transfer to the face and body for over ten years, the progression of transplanting a patient’s own fat cells to the breast is a fairly new technique, but no different than what surgeons have performed for years.  The key to a good result with this procedure is the surgeon’s experience and technique. Surgeons must handle the cells with the utmost of care for the transplant to take.  

Fat transplant consists of harvesting fat from one part of your body utilizing a small syringe. Then, carefully re-injecting these harvested cells into your breasts. There are many advantages of utilizing your own fat cells versus implants. 

  • Minimal incisions mean minimal scarring
  • Fat is real to your body,  where implants are not
  • The breast can be sculpted and shaped
  • Fat grafting to the breasts can help disguise breast implants

Fat transplant to the breasts can take from four to six hours. The fragile fat cells must be harvested slowly and gently. The fat is then placed into the breast not only so that it will create as aesthetic, natural appearance, but also so that the newly transplanted fat has a chance to survive.

As with any breast procedure, calcifications and lumps can occur.  Patients will need to continue their breast exams and annual mammograms.  No evidence has been found that shows that fat grafting to the breast is less safe than any other breast augmentation procedure.  Studies have begun over the last few years to study fat transplant to the breast.

Obviously, surgeons with extensive experience in grafting large volumes of fat to the body are most likely to obtain the best results.
